notes to referrers

Please note when you make a referral we need the following information about your patient:

  • Name and email address
  • Contact phone number
  • DOB (Date Of Birth)
  • DOI (if applicable)
  • DOS (if applicable)
  • ACC claim number (or identify other funder if not ACC)
  • NHI number (helpful but not essential)
  • Brief description of injury or diagnosis
  • Preferred orthotic treatment options
